Crooked Uneven Gum Line Perio SurgeryGummy Smile Rehabilitation - Crown Lengthening Gingiva Reshaping and ContouringOur patient is a 20 something year old female who had a lifelong history of having teeth that were not uniform in size and created a gummy smile appearance that had a juvenille characteristic.
She readily admitted to feeling embarrased to smile. She was very self conscious to the extent that it affected her social activities, at times.She had consulted with her General Dentist, a doctor with a successful record of creating true cosmetic and reconstructive makeovers, learning that the best choice treatment for her cosmetic goals would include hand crafted porcelain crowns. The patient was referred to Dr. Nemeth to undergo periodontic procedures for treating the excess gum tissue problems. Crown lengthening (wherein underlying bone tissue is selectively removed) combined with gum recontouring would be used to establish symmetrical relationships of exposed tooth structure across her upper arch. Obtaining an optimal fit for ceramic crowns always requires substantial tooth structure area that is properly prepared for achieving a superior bond. The periodontic crown lengthening procedures provided by Dr. Nemeth assured this requirement. 
In a step by step fashion, Dr. Nemeth treated each affected tooth in a sequence that created a balance of natural appearing gingival arches for the entire upper arch. This process eliminated the crooked gumline the patient felt she had.IV sedation was provided to the patient to achieve the highest degree of comfort for the patient. Dr. Nemeth used a scalpel and special high speed burr, as needed to trim varying amounts of soft and hard tissues on the affected teeth. Following an approximate 2 weeks of healing the patient was ready for the final porcelain restorations. The final outcome was very dramatic for the patient. As shown in the 2nd closeup photo, the patient achieved a gum line that appears natural and is symmetrical across the entire upper arch. In terms of aesthetics, her smile no longer has a juvenille appearance. The patient's new adult smile had a very strong impact for her. New levels of self confidence emerged almost overnight. The before and after full face photos are paired here to demonstrate the dramatic effects that are possible with specialized periodontic lip repositioning procedures.
